What are the responsibilities and job description for the Community Health Worker Navigator – Protecting Oregon Farmworkers position at Northwest Family Services?
This position will focus on providing healthcare education and outreach services to farm workers and their families. Connect farm workers and their dependents to wraparound services such as food, housing, transportation, childcare, etc.
Primary Job Functions
· Provide healthcare education and outreach services to farm workers.
· Provide participants in Clackamas, Multnomah, and Washington counties with enrollment, redetermination, and navigation services.
· Follow up with participants as needed.
· Maintain participant documentation and enter project data.
· Drive to different county areas in order to meet with participants.
· Regular attendance is required.
· Other duties as required.
Education / Experience
· Bilingual and bi-cultural English and Spanish is required.
· A high school diploma or equivalent is required.
· Must be a Certified OHA Assister or become certified within 6 months of taking the position.
· Have a Community Health Worker certification or become certified within 6 months of taking the position.

Pay: $25.00 - $27.00 per hour
