Overview
Assist patients in their homes by providing individualized nursing care under the supervision of Registered Nurses.
Monitor patient progress, report health changes, and adjust care plans accordingly.
Deliver care with empathy, coordinate with interdisciplinary teams, and utilize technology to maintain accurate electronic medical records.
Educate patients and families on health management and end-of-life care.
Travel within the community to support patients, manage caseloads efficiently, and ensure continuity of care.

Qualifications
- Valid LPN license and registration in New York State
- High School Diploma or LPN program completion
- Preferred minimum of one year home care or hospice experience
- Bilingual skills and driver’s license may be required based on operational needs
- Provide personalized, quality-driven nursing care and care management
- Communicate effectively within teams and across providers
- Use technology to document and coordinate patient care
- Support patient education and navigate complex health needs
