🩺 Key Responsibilities:

• Deliver skilled nursing care including medication administration, wound care, IV therapy, and catheter care

• Conduct thorough assessments and develop individualized care plans

• Monitor patient progress and respond to changes in condition with clinical accuracy

• Educate patients and family members on care plans, medications, and health management

• Document all care provided in compliance with regulatory and agency standards

• Uphold the highest standards of patient care, safety, and professional ethics

• Collaborate effectively with interdisciplinary teams and external providers

🎯 Qualifications:

• Active LPN License in the State of New York

• Minimum 1–2 years of nursing experience (home care, pediatrics, or geriatrics preferred)

• Strong clinical judgment and the ability to work independently

• CPR/BLS certification (required)

• Proficient in electronic health records (EHR) systems (preferred)

• Excellent communication, organizational, and interpersonal skills

• Reliable transportation and/or valid driver's license preferred
