Salary Range: $102,107 - $135,000 (based on years of exp)

Sign on Bonus: $10K

Relocation Bonus: as per the distance from the facility.

Job Summary and Qualifications

• The Facility Case Management Director oversees and coordinates all department activities, ensuring compliance with policies and regulatory standards while integrating patient care and discharge planning across departments.
• Responsible for continuity of care, optimal resource utilization, and achieving outcomes through interdisciplinary collaboration and stakeholder engagement.
• Acts as a subject matter expert with strong leadership, communication, and change management skills, while managing multiple priorities with minimal supervision.

Responsibilities

• Directs and evaluates departmental operations including case management models, staffing, onboarding, competencies, and use of information technology.
• Allocates resources effectively to meet productivity and quality goals.
• Oversee core case management functions and ensure adherence to care coordination and discharge planning processes.
• Collaborate with Facility CFO and Division CM Leader to meet organizational goals.
• Monitors performance, analyzes data, and implements sustainable improvements.
• Ensures compliance with policies, SOPs, and maintains strong relationships with key stakeholders (CMO, CNO, Legal, etc.).
• Upholds Code of Conduct and Mission & Value Statement; performs additional duties as assigned.

Qualifications

• Bachelor’s degree required; master’s in nursing, Health Administration, or Business Administration preferred.
• Required licensure: RN, LCSW, or LMSW.
• Minimum 3 years in acute care hospital case management and 2 years in leadership roles.
