- Lead and manage all operations of the case management department
- Oversee staffing, onboarding, and staff competencies
- Ensure effective care coordination and discharge planning
- Monitor performance metrics and implement quality improvement initiatives
- Ensure compliance with organizational policies and regulatory standards
- Collaborate with senior leadership and interdisciplinary teams
- Support staff in complex cases and drive operational performance
- Promote a positive, collaborative, and inclusive team culture
- Bachelor's degree required; Master's degree in Nursing, Healthcare Administration, or Business Administration preferred
- RN, LCSW, or LMSW license required
- 3 years of acute care hospital case management experience
- 2 years of case management leadership experience
- Strong understanding of clinical operations, discharge planning, and hospital workflows
- Excellent leadership, communication, and team-building skills
- Ability to manage multiple priorities, adapt quickly, and make independent decisions
