Seeking a licensed social worker to coordinate care for a defined population, collaborating with healthcare teams to ensure appropriate, cost-effective services that promote independence and improve quality of life.



Key responsibilities include:

  • Conducting psychosocial assessments, identifying problems, and providing counseling, referrals, and community resource access.
  • Developing and monitoring personalized care plans in collaboration with healthcare team members.
  • Advocating for resource utilization, documenting activities accurately, and analyzing outcomes for continuous improvement.
  • Assisting members with applications for programs like Medicaid and Medicare, and supporting end-of-life planning and legal processes.
  • Providing education and support to members, families, caregivers, and healthcare staff, maintaining current knowledge of social work standards and regulations.
  • Scheduling family meetings, completing guardianship paperwork, and assisting with virtual court proceedings for inpatient cases.

Qualifications include:

  • Minimum one year of clinical social work experience in healthcare.
  • Master’s degree in social work (MSW).
  • Licensure as an Independent or Graduate Social Worker within six months of hire, depending on the state.

Preferred skills:

  • Experience with Windows-based software and community resources.
  • Knowledge of regional regulatory issues.
