- Conducting psychosocial assessments, identifying problems, and providing counseling, referrals, and community resource access.

- Assisting members with applications for programs like Medicaid and Medicare, and supporting end-of-life planning and legal processes.

- Minimum one year of clinical social work experience in healthcare.
