- Performing psychosocial assessments, problem identification, and providing counseling and education to members, families, and healthcare teams.
- Managing caseloads by developing and monitoring personalized care plans to promote independence and improve quality of life.
- Facilitating access to community resources, legal processes, and financial counseling, while ensuring timely and cost-effective service delivery.
- Documenting activities in compliance with standards and analyzing outcomes for quality improvement.
- Assisting with applications for government-funded programs and addressing issues like abuse/neglect referrals.
- Coordinating family meetings, completing guardianship paperwork, and providing technology support for virtual court attendance (Inpatient only).
- Minimum one year of clinical social work experience in healthcare.
- Masters degree in social work (MSW).
- Licensure from relevant states, including requirements for independent or graduate social work credentials.
- Experience with Windows-based software programs.
- Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
