As a vital part of our Adult Program Nursing Team, you'll collaborate with nurses, residential staff, medical providers, and families to deliver holistic, dignified care. Location: Sandwich, MA
Compensation: $24.00-$25.20 per hour (based on experience)
Schedule: Full-time, with occasional on-call responsibilities
Key Responsibilities:
- Schedule and coordinate medical and dental appointments
- Transport individuals to appointments, communicate medical information to providers, and complete related paperwork and follow-ups
- Assist with personal care tasks including changing, toileting, wound care, and laundry, while promoting independence
- Report medical and health concerns to the Nurse Manager and collaborate with the Adult Services Nursing Team, program managers, and staff
- Ensure high-quality care in compliance with regulations and individual needs
- Assist with administering prescribed medications and treatments
- Manage and replenish medical and first aid supplies
- Maintain organized and up-to-date medical files
- Document vital signs, data, and relevant information accurately
- Participate in team meetings as needed
- Meet regularly with the AS Nurse Manager and Nursing Team to review patient care
- Perform other related duties as assigned by the supervisor
- High School Diploma or GED
- Valid certification as a Nurse's Aide
- Prior CNA experience preferred but not required
- Experience with adults with developmental disabilities and dementia care preferred but not required
- Ability to physically intervene in instances of extreme behavior
- Ability to lift 50 pounds
- Valid driver's license
