Job Description:
- Provide compassionate hospice and palliative care to patients and families in home and facility settings.
- Administer medications and treatments according to physician orders and LPN standards of practice.
- Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans.
- Conduct patient assessments and accurately document observations and interventions.
- Ensure safe handling, storage, and administration of controlled medications.
- Participate in care coordination meetings and continuous quality improvement activities.
- Uphold the mission and values of Hawaii Care Choices while delivering high-quality patient-centered care.
- Current Hawaii LPN license and valid CPR certification required.
- Minimum of one year of nursing experience, preferably in hospice or palliative care.
- Strong clinical assessment, communication, and critical-thinking skills.
- Ability to manage patient care independently across multiple locations (home-to-home travel required).
- Sensitivity to cultural, religious, and emotional needs of patients and families.
- Excellent organizational skills and attention to detail in documentation.
- Professional demeanor with consistent maintenance of appropriate boundaries.
- Flight covered.
- Mileage reimbursed.
