Schedule: Monday – Friday, 8:00 AM to 5:00 PM; must be available for alternate weekends if needed.

Job Description:
- Perform comprehensive patient assessments and document health data at each visit.
- Develop and update individualized treatment plans in collaboration with providers.
- Implement preventive and rehabilitative nursing interventions safely and on time.
- Manage assigned cases, coordinate with internal teams, and make referrals as needed.
- Supervise and provide input to HHAs and LPNs per agency policy.
- Educate patients, families, caregivers, and staff on care plans and instructions.
- Prepare accurate, timely electronic documentation to support reimbursement.
- Maintain communication with providers, caregivers, and external agencies to ensure continuity of care.
- Valid U.S. driver’s license, vehicle, and insurance.
- Clean driver’s abstract.
- Current BLS certification.
- Strong clinical judgment and problem-solving skills.
- Ability to manage workload across various geographic areas.
- Strong communication and interpersonal skills.
- Ability to safely use home-health equipment (BP cuffs, adaptive devices, lifts, etc.).
- Mileage reimbursement.
