About Us
MediSys Family Care Center in Jamaica, NY provides essential specialty medical services, including Internal Medicine, Pediatrics, and Podiatry. We are looking for a compassionate and dedicated Social Workers (LMSW/LCSW) to support our patients and enhance their overall well-being.
Key Responsibilities
- Assess patients’ social, emotional, and financial needs to identify barriers to care.
- Provide therapy and short-term counseling to help patients and families cope with illness and stress.
- Advocate for patients by connecting them with healthcare services and community resources.
- Collaborate with medical professionals to create comprehensive care plans and ensure smooth care transitions.
- Assist with discharge planning, including follow-up care and home health arrangements.
- Offer crisis intervention and emotional support as needed.
- Educate patients and families on medical conditions, treatment options, and coping strategies.
