REGISTERED NURSE – HOSPICE

POSITION OVERVIEW:
As a Hospice RN, you will play a critical role in supporting patients during their final journey, focusing on comfort, dignity, and quality of life. You will assess patient needs, manage symptoms, and provide emotional support while collaborating closely with an interdisciplinary care team.
KEY RESPONSIBILITIES:
- Conduct regular patient assessments, monitoring physical and emotional well-being.
- Administer medications, treatments, and provide effective pain and symptom management.
- Deliver emotional and psychosocial support to patients and their loved ones.
- Educate families about the hospice philosophy, care expectations, and end-of-life processes.
- Coordinate care with physicians, social workers, chaplains, and home health aides.
- Manage medical equipment and supplies, ensuring patient comfort at all times.
- Maintain accurate and timely documentation of care plans and patient progress.
- Provide crisis intervention and bereavement support as needed.
