Job Title: Ambulatory Medical Case Manager
Pay Rate: $40/hr - $45/hr W2
Position Details:
Location: Cambridge, MA – Fully Onsite
Schedule: Monday–Friday, 40 hours/week
- 8:30 AM–5:00 PM OR
- 8:00 AM–4:30 PM
Required Licensure: Massachusetts LICSW or LCSW
The Ambulatory Medical Case Manager will provide comprehensive case management services to identified patients in an outpatient medical setting. The role involves researching patient needs, conducting patient interviews, monitoring care utilization patterns, and developing individualized care plans.
The Case Manager will collaborate closely with clinical staff and treating clinicians to develop comprehensive outpatient care plans, communicate areas of concern, and provide regular clinical updates.
The position will also contribute to continuous improvement of case management practices, educate medical staff on applicable criteria and guidelines, identify potential quality-of-care concerns, and assist in developing appropriate solutions.
Key Responsibilities:
- Provide ambulatory case management services to patients.
- Conduct patient interviews and assess individual care needs.
- Monitor care utilization patterns and develop appropriate care plans.
- Collaborate with physicians, clinicians, and other healthcare professionals.
- Communicate patient concerns and clinical updates to treating clinicians.
- Manage a caseload of approximately 40–50 patients.
- Monitor and coordinate hospitalizations.
- Conduct advance directive outreach.
- Identify potential quality-of-care issues and assist with developing solutions.
- Educate medical staff regarding case management criteria and guidelines.
- Contribute to process and quality improvement initiatives.
- Assist with developing reports, resources, and materials for other departments.
- Apply case management principles to support effective patient care.
- Maintain accurate documentation and communicate effectively with cross-functional healthcare teams.
Required Qualifications:
- Master's degree in social work or a related field.
- Active Massachusetts LCSW or LICSW license.
- Minimum 3 years of social work experience in a medical/healthcare setting.
- Familiarity with managed care and case management principles.
- Experience working with medically complex:
- Children
- Adults
- Geriatric patients
- Strong organizational and analytical skills.
- Excellent problem-solving and interpersonal skills.
- Strong written and verbal communication skills.
- Ability to manage multiple cases and priorities in a fast-paced healthcare environment.
