Responsibilities:
• Assess the social and emotional needs of residents and develop individualized care plans to meet those needs
• Provide counseling, guidance, and support to residents and their families to address concerns and improve overall well-being
• Coordinate with interdisciplinary teams to ensure resident-centered care and promote positive outcomes
• Facilitate group activities and therapies to enhance socialization and engagement
• Maintain accurate records and documentation of resident interactions and care plans
• Collaborate with community resources and outside agencies to access additional support services for residents
Requirements:
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
• Commitment to providing high-quality care and services to residents.
• Active Massachusetts License (LICSW, LCSW, LSW)
Salary : $70,000 - $85,000
