Position Summary:
This is a field-based role for a Certified Nurse Practitioner (CNP or AGNP) interested in wound care and long-term primary care. No certification in wound care? No problem. We’ll help you get there—and cover the cost.
Key Responsibilities:
- Visit multiple senior care sites each week
- Deliver in-place wound evaluations and follow-up care
- Participate in care coordination and medication reconciliation
- Build trusted relationships with residents, families, and facility teams
Qualifications:
- Current MN CNP license
- Wound care certification or willingness to obtain
- Background in long-term care or family/geriatric practice
- Must have reliable transportation
