Pay: $90,000 – $130,000 DOE

Location: Serving the Greater Twin Cities Metro Area (21 County Service Area)

What You'll Do:
- Deliver skilled, holistic nursing care for end-of-life patients at home
- Educate and empower families through the hospice journey
- Coordinate with physicians, social workers, and aides to develop care plans
- Guide discussions on advance directives and end-of-life choices
- Proactively manage symptoms and adjust care as needed
- Maintain accurate, timely documentation in line with hospice regulations
What You Bring:
- Active RN license in Minnesota
- Hospice or oncology experience (required)
- Deep compassion and strength to support patients and families during emotional times
- Clinical autonomy, critical thinking, and great communication skills
- Experience in an interdisciplinary care model
