Job Title: Registered Nurse (RN) Case Manager – Home Health Specialist

Company: Home Health Care, Inc.

Location: Greater Twin Cities Metro Area

Schedule: Full-Time (Monday to Friday, 8:00 AM – 5:00 PM), 1 weekend/month, 1 on-call shift/week

Compensation: $85,000 – $130,000 annually $6,000 in bonuses during first year of employment

Responsibilities:

• Lead comprehensive nursing assessments and individualized care plans
  
• Conduct OASIS visits and coordinate services across care settings
  
• Collaborate with physicians and providers to maintain clinical continuity
  
• Deliver education, emotional support, and transition planning for families
  
• Ensure accurate documentation and regulatory compliance
  

Qualifications:

• MN RN license in good standing
  
• 3 years of clinical RN experience; home health preferred
  
• Strong documentation, care planning, and assessment skills
  
• Reliable vehicle and valid driver’s license
