Physical Therapist – Home Health (Full-Time)
Schedule
- Monday–Friday, 8:00 AM – 5:00 PM
- No weekends or on-call requirements
Coverage Area
- Harrison, Marion, Taylor, Barbour, Upshur, Lewis, and Doddridge counties
Compensation & Requirements
- Mileage reimbursement: $0.54 per mile
- Active West Virginia Physical Therapist license required
Preferred Qualifications
- Home Health experience required/preferred
- Experience with OASIS documentation
- Experience with HCHB (Homecare Homebase) is a plus
Position Overview
The Physical Therapist provides evaluation and treatment services to patients in a home health setting, following physician orders and referrals. This role is focused on improving patient independence, mobility, and overall functional outcomes through personalized care plans.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations using appropriate assessment tools and techniques
- Review patient medical history, diagnoses, precautions, and contraindications
- Develop individualized treatment plans with measurable, functional goals
- Reassess patient progress regularly and adjust treatment plans as needed
- Communicate effectively with physicians, care team members, patients, and families regarding patient status and progress
- Collaborate with medical providers on treatment plans, progress updates, and discharge planning
- Recommend and assist patients in obtaining adaptive equipment to improve functionality
- Maintain accurate, timely clinical, billing, and compliance documentation in accordance with all regulatory requirements
- Adhere to all applicable professional standards, policies, and federal/state regulations
- Supervise Physical Therapist Assistants and therapy aides in accordance with state practice laws
- Participate in staff meetings, ongoing training, and continuing education to maintain clinical excellence
