Licensed Practical Nurse (LPN) – Home Health
Shift Requirements: Monday–Friday, 8:00 AM – 5:00 PM
On-Call: 1 weekend per month
License Requirement: Active West Virginia LPN License
Productivity Requirement: 32 points per week
Mileage Reimbursement: Yes
Coverage Area: Monongalia, Preston, Wetzel, Grant, and Tucker Counties
Preferred Experience: Prior home health experience
Position Overview:
The Licensed Practical Nurse (LPN) provides direct patient care in accordance with individualized care plans while promoting patient dignity, independence, and overall well-being. This role involves working collaboratively with interdisciplinary teams to ensure high-quality, patient-centered care in a home health setting.
Key Responsibilities:
- Deliver patient care in accordance with established care plans and physician orders
- Monitor vital signs and report any abnormal findings
- Administer medications, treatments, oxygen therapy, and specialized care (e.g., ostomy care) as required
- Perform routine care in coordination with PT, OT, and Speech Therapy plans
- Maintain and enforce infection control standards and report any concerns
- Accurately document all patient care, observations, and communications
- Monitor patient progress and update care plans as needed
- Order and manage medical supplies, medications, and equipment
- Ensure all patient needs are met in accordance with physician instructions
- Respond to and document incidents appropriately, including patient assessment and reporting
- Ensure medical equipment is functioning properly and coordinate repairs with DME providers
