Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process
- Assess patient status, develop care plans, implement interventions, and evaluate outcomes
- Supervise and direct activities of various levels of assigned nursing staff; delegate tasks as appropriate
- Coordinate care with other disciplines and members of the interdisciplinary team
- Apply critical thinking, professional judgment, and independent decision-making in day-to-day care
- Document care accurately and in a timely manner, ensuring safety and quality standards
Requirements
- Ability to provide direct nursing care in a home healthcare - hospice setting
- Strong critical thinking, clinical judgment, and problem-solving skills
- Effective communication and collaboration with an interdisciplinary team
- Supervisory experience or demonstrated ability to oversee nursing staff
- Must be able to work days (12-hour shifts) and meet travel assignment requirements
- Active California RN license (as applicable to this role)
