Responsibilities
- Perform PT evaluations and develop individualized treatment plans for adolescents, adults, and geriatrics in outpatient and inpatient settings, including post-operative ortho (hip, knee, shoulder) and spine cases
- Address spinal conditions and musculoskeletal pain (low back and neck), with progressive progression of therapy as clinically indicated
- Provide neuro rehabilitation for conditions such as stroke, TBI, SCI, MS, and Parkinson’s disease; implement and monitor home exercise programs
- Educate patients and families, provide written/guided home exercise plans, and document care in the patient record
- Collaborate with an interdisciplinary team to coordinate care, participate in rounds as appropriate, and ensure appropriate patient discharge planning
- Maintain accurate, timely charting in the designated electronic health record (Epic noted in compliance details)
Requirements
- Licensure: Active physical therapy license required; pending license accepted (processing 10–12 weeks)
- Certifications: BLS required
- Experience: Minimum 1 year of clinical experience
- Setting experience: Ability to work in outpatient and acute care/ SNF/LTC settings
- Additional: Permanent address must be at least 50 miles from the facility; willingness to travel and support required coverage; on-site duties may include some inpatient care and SNF coverage
- Documentation: Charting experience required; Epic noted as the system
