Responsibilities
- Administer prescribed medications and treatments per physician orders and facility protocols
- Monitor residents’ vital signs, assess changes in condition, and report to the nursing team
- Provide direct patient care and activities of daily living support; assist residents with mobility and orientation
- Perform basic nursing tasks including wound care, injections as allowed by scope of practice, and routine care planning
- Document accurately in the electronic medical record and communicate updates to the registered nurse and interdisciplinary team
- Float to other units as needed and deliver compassionate care to residents with dementia
Requirements
- Current Licensed Practical Nurse license (state of Vermont) or eligibility to practice as an LPN
- BLS through American Heart Association required
- Minimum 3 years of nursing experience; LTC experience required
- Prior travel nursing experience preferred
- Ability to float to other units as needed; comfort working with residents with dementia
- Completion of an approved practical nursing program
