Responsibilities
- Provide direct nursing care per the plan of care for residents in a long-term care setting
- Administer prescribed medications and document administration per facility policy
- Monitor residents’ condition, perform routine assessments, and report changes to the supervising RN
- Assist with activities of daily living and maintain safety, dignity, and infection control
- Collaborate with the interdisciplinary team and document care plans and outcomes
Requirements
- Active LPN license in Vermont or eligibility to practice in Vermont
- Completion of an approved LPN program (diploma/degree)
- Experience in long-term care or geriatric nursing preferred
- Ability to work all shifts and participate in a travel assignment
- Strong communication, documentation, and teamwork skills
