Responsibilities

• Conduct resident evaluations to determine PT goals and develop individualized treatment plans
• Implement therapeutic interventions to improve mobility, strength, balance, gait, ROM, and activities of daily living
• Monitor progress with regular reassessments, adjust plans as needed, and document outcomes clearly
• Collaborate with nursing, occupational therapy, speech therapy, and social work to support comprehensive care and discharge planning
• Educate residents and families on home exercise programs, safety strategies, and progress expectations
• Ensure documentation complies with regulatory standards and facility policies
  
  

Requirements

• Ability to work in a long-term care setting and adapt to an interdisciplinary team
• Strong documentation, clinical reasoning, and communication skills
• Flexibility to work the posted shift and 35 guaranteed hours per week
• Ability to travel to the assignment location for the duration of the contract
• Demonstrated professionalism and reliable attendance in a fast-paced environment
