Responsibilities

• Provide direct patient care to residents in a long-term care setting under supervision of a registered nurse
• Administer medications and treatments as prescribed, ensuring accuracy and timely documentation
• Monitor vital signs, assess resident condition, and report changes promptly to the supervising RN
• Assist residents with activities of daily living (ADLs) and promote safety, comfort, and dignity
• Document care provided and update care plans or logs in the electronic medical record as required
• Collaborate with the interdisciplinary team to coordinate resident care and discharge planning as needed
  
  

Requirements

• Active Licensed Practical Nurse (LPN) license in North Carolina
• Graduation from an approved practical nursing program
• Ability to work evening shifts (3:00 PM–11:30 PM) and every other weekend
• Effective communication skills and ability to document accurately
• Prior experience in a long-term care or geriatric care setting preferred
• Compliance with facility health requirements and orientation processes
