Responsibilities

• Provide direct patient care, including assistance with activities of daily living and mobility
• Administer medications and treatments per care plan and document administration
• Monitor patient status, vital signs, and changes; report to RN or supervising clinician
• Assist with wound care, injections, and routine therapies as directed
• Maintain accurate documentation and uphold standard precautions and infection control practices
  
  

Requirements

• Active Vermont Licensed Practical Nurse license
• Current CPR/BLS certification
• Graduation from an approved practical nursing program
• Prior experience in long-term care or skilled nursing setting preferred
