Responsibilities

• Conduct initial screenings and comprehensive evaluations of residents with speech, language, voice, swallowing, or cognitive-communication concerns
• Develop and implement individualized treatment plans; deliver direct therapy and adjust plans based on progress
• Document all therapy sessions, progress notes, and outcomes in accordance with regulatory and facility requirements
• Collaborate with an interdisciplinary care team (nursing, dietary, rehabilitative therapies) to coordinate care and discharge planning
• Educate residents, families, and caregivers on communication strategies, compensation techniques, and safe swallowing precautions

• Availability for a 13-week travel assignment starting May 4, 2026
• Day shift schedule: 08:00–16:30, five days per week
• Ability to work in a skilled nursing facility environment
• Strong documentation, communication, and interprofessional collaboration skills
