About This Role
Own your participants' care in the community as a core member of PACE-RI's interdisciplinary team. Assess safety and acute care needs in the home, reconcile medications, manage safe transitions of care, and build care plans alongside the PCP and social worker. Triage participant and family calls, coordinate home services with the scheduler, and supervise and teach the CNAs delivering personal care: real clinical ownership and autonomy. Rotating weekends and holidays. PACE is Rhode Island's nonprofit all-inclusive care program for seniors, and this is a PSLF-qualifying employer. Pay starts at $38/hr with real education support: $3,000 and 40 hours annually for continuing education, licensure renewal reimbursement, and CPR certification covered. Separate work phone provided. Supportive, fast-paced team where the COO is a nurse, and work-life balance is taken seriously. New grads welcome. Requirements: active RI RN license, CPR, associate degree (BSN preferred), driver's license and reliable transportation. Experience preferred but not required.
