About This Role
Own your participants' care across PACE-RI's Northern Rhode Island territory, based out of the Woonsocket center. Assess safety and acute care needs in the home, reconcile medications, manage safe transitions of care, and build care plans as a core member of the interdisciplinary team alongside the PCP and social worker. Triage calls, coordinate home services, and supervise and teach CNAs: real clinical ownership and autonomy. Rotating weekends and holidays. PACE is Rhode Island's nonprofit all-inclusive care program for seniors, and this is a PSLF-qualifying employer. Pay starts at $38/hr with real education support: $3,000 and 40 hours annually for continuing education, licensure renewal reimbursement, and CPR certification covered. Separate work phone provided. Supportive, fast-paced team where the COO is a nurse, and work-life balance is taken seriously. New grads welcome. Requirements: active RI RN license, CPR, associate degree (BSN preferred), driver's license and reliable transportation. Experience preferred but not required.
