- Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
- ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Duties
- Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Develop individualized care plans by collaborating with physicians, specialists, and other healthcare professionals.
- Coordinate discharge planning and post-acute care arrangements to ensure smooth patient transitions across settings.
- Manage utilization review and management processes to optimize resource use while maintaining quality standards.
- Monitor patient progress through diagnostic evaluations and ongoing assessments, adjusting care plans as necessary.
- Document all patient interactions, medical records, and case management activities accurately in compliance with HIPAA regulations.
- Provide education and support to patients and families regarding treatment options, medication management, and health maintenance strategies.
Requirements
- Valid registered nurse license with current state certification.
- Critical care experience within hospital settings such as ICU or emergency departments is essential.
- Experience working in Level I or Level II trauma centers, including trauma and emergency medicine environments.
- Background in hospital medicine, infectious disease care, or behavioral health is highly desirable.
- Strong knowledge of physiology, medical terminology, and acute care protocols.
- Proven ability to perform comprehensive patient assessments and discharge planning efficiently.
- Familiarity with medical records systems, diagnostic evaluation procedures, and outpatient clinic operations.
- Excellent communication skills for effective collaboration with multidisciplinary teams and patient education.
