Registered Nurse Case Manager – Hospice
Location: Canonsburg, PA Schedule: 8 AM – 5 PM, Monday–Friday | 6 units daily / 30 units weekly On‑Call: 1 weekend per month 1 evening per week Territory: Canonsburg and approximately 15–25 miles surrounding the community Travel: Local travel across multiple counties required
OverviewThis is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a meaningful impact in the lives of patients receiving hospice care. In this role, you will coordinate care, support families, and serve as a clinical leader within a compassionate, patient‑focused team. The ideal candidate is organized, empathetic, and confident managing care in a field‑based environment.
Key Responsibilities- Manage and oversee patient care within hospice settings, ensuring high‑quality, individualized care
- Develop, implement, and evaluate patient‑specific care plans
- Collaborate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
- Provide leadership, mentorship, and guidance to nursing staff
- Maintain accurate and timely clinical documentation
- Communicate effectively with patients, families, and caregivers
- Participate in on‑call rotation (one evening weekly one weekend monthly)
- Travel locally to patient homes and facilities across multiple counties
- Education: High School Diploma or GED
- Experience:
- 2 years of Hospice experience
- 2 years of Management or leadership experience
- 1 years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health
- Licensure:
- Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
- Certifications (ALL required):
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- Other Requirements:
- Reliable transportation and ability to travel locally
- Strong time‑management and self‑organization skills
- Ability to work independently in field‑based environments
