OverviewThis role offers a meaningful opportunity for a compassionate Registered Nurse Case Manager to support patients and families navigating hospice care. You will serve as a clinical leader, coordinating care plans, guiding nursing staff, and ensuring patients receive exceptional, dignified end‑of‑life care. Key Responsibilities- Manage and oversee patient care within hospice settings, ensuring high‑quality, patient‑centered services
- Develop, implement, and evaluate individualized care plans
- Coordinate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
- Provide leadership, mentorship, and guidance to nursing staff
- Maintain accurate, timely clinical documentation
- Communicate effectively with patients, families, and caregivers
- Participate in on‑call rotation (one evening weekly one weekend monthly)
- Travel locally to patient homes and facilities across multiple counties
- Education: High School Diploma or GED
- Experience:
- 1 years of Hospice experience
- 1 years of Management or leadership experience
- 1 years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health
- Licensure:
- Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
- Certifications (ALL required):
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- Other Requirements:
- Reliable transportation and ability to travel locally
- Strong time‑management and self‑organization skills
- Ability to work independently in field‑based environments
