Role Description
This is a full-time, on-site role for a Registered Nurse Case Manager providing care to patients in the area east of the city of Pittsburgh (Monroeville, Murrysville area)The Registered Nurse Case Manager will coordinate and oversee the care of hospice patients, including providing direct patient care, developing and implementing care plans, and collaborating with interdisciplinary teams. Additional responsibilities include conducting patient assessments, monitoring their conditions, and educating patients and families on care plans, while ensuring compliance with regulations and standards of care.
Qualifications
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Experience in hospice or palliative care, or in related fields such as case management or home health care
- Strong skills in patient assessment, care planning, and symptom management
- Proficiency in collaborating within interdisciplinary teams and communicating effectively with patients and families
- Basic skills in documentation and understanding of healthcare compliance regulations
- Compassionate approach with excellent emotional intelligence, and ability to provide supportive care in end-of-life situations
- Valid driver's license and reliable transportation for patient visits
