Key Responsibilities
- Collaborate with the interdisciplinary team (IDT) to develop and implement individualized care plans
- Complete and assist with MDS documentation in accordance with regulations
- Assess, treat, and manage wounds and skin conditions
- Monitor, prevent, and manage pressure ulcers and infections
- Perform routine skin assessments and support pain management efforts
- Educate residents and families on wound care treatments and prevention strategies
- Evaluate nutritional needs related to wound healing
- Recommend appropriate wound care treatments, devices, and products
- Ensure compliance with all facility policies, procedures, and regulatory standards
- Report any concerns related to abuse, neglect, or resident safety
- Active, unencumbered RN or LPN license in the state of Florida
- Minimum of 2 years of nursing experience (wound care experience preferred)
- Strong clinical assessment and documentation skills
- Excellent communication and teamwork abilities
- Experience in skilled nursing or long-term care is a plus
