Essential Functions Of Job:
Qualifications:
Minimum Education, Experience and Training: Six months of experience as a Human Services Case Manager, or County Human Services Intern; or one year of experience in public or private social work and a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or a bachelor's degree with a social welfare major; or any equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
Currently County Employees will follow the County Promotion/Demotion Policy.

OVERALL OBJECTIVE OF JOB: Advocate for consumers of LTC addressing concerns about care, resident rights and quality of life.
Essential Functions Of Job:
- Assist consumers of long-term care services with concerns about care, resident rights, and quality of life issues by conducting visits to investigate and seek resolution at the residents’ request and direction.
- Conduct Quality Assurance visits on a regular basis to nursing homes and personal care homes to maintain a visible presence.
- Interpret federal and state regulations for Personal Care homes and Nursing facilities to ensure compliance and refer to appropriate government agencies when beyond the scope of the Ombudsman program.
- Provide staff in-service programs as requested by area long term care facilities.
- Develop and present resident rights programs to consumers of long-term care.
- Provide information and assistance to consumers, families of consumers and staff of long-term care facilities as requested.
- Mentor Volunteer Ombudsmen with joint facility visits and quarterly meetings.
- Maintain proper documentation of Ombudsman activities in Ombudsmanager.
- Develops and manages ongoing Ombudsman volunteer retention activities.
- Develops and conducts PEER trainings and provides ongoing support to PEER programs.
- Complete initial and ongoing Ombudsman training.
- Provide information, assistance and referral services, including the completion of applications and forms.
- Interpret and/or explain agency programs, federal, state and county policies and procedures for individuals, groups and agencies.
- Travel as necessary, including meeting with and assisting individuals in accessing needed support and services.
- Prepare studies and reports as necessary.
- Attend meetings, training, and seminars as necessary.
- Performs other job-related duties as may be assigned.
