Case Manager
Location: Orlando, FL (on-site)
Schedule: Monday–Friday | 8:00am–5:00pm
Compensation: $22/hour (direct hire)
About The Role
This is a community health clinic–based Case Manager / Social Worker position supporting diverse patient populations, including individuals managing chronic health conditions and ongoing care needs.
The role focuses on care coordination, patient navigation, and advocacy, helping patients stay engaged in treatment and overcome barriers to care in an outpatient setting.
Key Responsibilities
Manage a caseload of approximately 50 patients
Support patients with care plan navigation, appointment scheduling, and follow-ups
Coordinate referrals with specialists and third-party providers
Connect patients to community resources (housing, transportation, insurance, benefits)
Advocate for patients to reduce barriers to care
Maintain accurate documentation and case notes in the EHR
Collaborate closely with providers and clinic staff
Ideal Background
Degree in Social Work, Human Services, Psychology, Public Health, or related field or equivalent healthcare case management experience
Prior experience in healthcare, clinic, or community health settings preferred
Strong communication, organization, and patient advocacy skills
Comfortable managing a high-volume caseload
Spanish or Creole bilingual strongly preferred
