- 8-Hour Evening Shift: 3:00 PM – 11:30 PM | 40 hours/week guaranteed
- 12-Hour Evening Shift: 11:00 AM – 11:00 PM | 36 hours/week guaranteed

Responsibilities
- Provide direct nursing care to residents in accordance with their individualized care plans.
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor residents' health conditions and report any changes to the supervising RN or physician.
- Assist with ADLs (Activities of Daily Living) and ensure residents’ comfort and safety.
- Document all care provided using the facility’s EMR system (PointClickCare).
- Collaborate with other healthcare professionals to ensure quality patient care.
- Maintain a clean and safe environment for residents.
- 1-3 years’ experience required.
- SNF (Skilled Nursing Facility) experience; LTC-only experience acceptable.
- EMR Experience: Prior use of PointClickCare EMR system preferred.
- Active LPN License in New York (NY)—Required
- BLS Certification (American Heart Association)—Required
- Will accept local candidates.
- Local pay rates apply to candidates residing within 60 miles of the facility.
