Responsibilities
- Deliver high-quality patient care by performing thorough assessments, vital sign monitoring, and physical examinations tailored to long-term care residents.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and pain management treatments.
- Manage complex medical needs such as catheterization, tube feeding, ventilator support, and wound care, ensuring adherence to aseptic techniques and infection control policies.
- Document all patient interactions meticulously using Electronic Health Record (EHR) systems like Epic or Cerner; maintain compliance with HIPAA and confidentiality standards.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans that address dementia care, Alzheimer's disease, behavioral health issues, and other chronic conditions.
- Assist with diagnostic evaluations such as specimen collection or processing and coordinate discharge planning for residents transitioning to other levels of care.
- Provide education and support to residents and their families regarding medication administration, disease management, and health coaching strategies.
Skills
- Proven experience working with geriatric patients in skilled nursing facilities or long-term care environments.
- Strong knowledge of long-term care regulations, dementia care practices, and memory support strategies.
- Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with ICD coding (ICD-9/10) is highly desirable.
- Critical care experience including ICU or emergency medicine is a plus for managing acute situations effectively.
- Skilled in IV insertion, catheterization, sterile processing techniques, and basic life support (BLS/CPR).
- Excellent patient assessment skills combined with compassionate caregiver communication tailored to individuals with disabilities or cognitive impairments.
- Ability to work efficiently within a multidisciplinary team while maintaining clinical confidentiality policies and adhering to long-term care regulations.
