Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
- Administer medications and treatments accurately, including injections, IV infusions, tube feedings, and wound suturing, adhering strictly to aseptic techniques and safety protocols.
- Monitor residents’ conditions closely using telemetry and other monitoring systems; respond promptly to acute changes or emergencies such as trauma or critical illness.
- Document all patient interactions and treatments precisely within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with physicians and interdisciplinary teams on diagnostic evaluations, medication adjustments, discharge planning, and complex case management involving behavioral health or memory care needs.
- Provide specialized care for diverse populations including geriatrics, individuals with developmental disabilities, hospice patients, and those requiring trauma or infectious disease management.
- Support residents’ activities of daily living (ADLs), including catheter care, ostomy management, and tube feeding while maintaining dignity and respect.
Qualifications
- Current licensure as an RN or LPN in good standing; additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Pediatric Advanced Life Support (PALS) are preferred.
- Proven experience working in long-term care facilities or nursing homes; experience with memory care or behavioral health is highly desirable.
- Familiarity with electronic health records (EHR) systems such as Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in medical documentation and ICD coding (ICD-10/ICD-9).
- Strong knowledge of medical terminology, physiology, aseptic techniques, infection control practices, and medication administration protocols.
- Ability to perform complex patient assessments including vital signs monitoring and laboratory specimen collection/processing.
- Experience working with individuals with disabilities or chronic conditions requiring specialized interventions like dialysis or ventilator management is advantageous.
- Excellent communication skills combined with critical thinking abilities to triage urgent situations effectively.
