Primary Work Location:
Full time remote or hybrid with occasional in person meetings at various Cardea Health locations across Alameda County and the city of San Francisco.
Job Summary:
The Intake Care Coordinator assists the RN Intake Coordinator and clinical staff across programs in collecting and synthesizing the necessary documents and information required for program admission and service authorization. The Intake Care Coordinator also assists with benefit connection, care coordination at Cardea Health sites and data entry and collection.
Referral and Bed Management:

Conduct preliminary screening of program referrals, identify gaps and missing documentation, and work with referring partners to gather needed information. Communicate completed referrals to Intake Coordinator RN for review

Complete referrals to adjacent community support for qualifying clients, including Enhanced Care Management and Housing and Community Supports

Refer unbenefited clients to Medi-Cal enrollment specialist team

Assist the Intake Coordinator RN with requests for Information for service authorization

Collect and synthesize data on referrals, admissions, and referral sources for monthly, quarterly, and annual reports

Assist the Intake Coordinator RN in the management of phone and email communication with referring partners

Assist the Intake Coordinator RN with maintenance of referral waitlist – communicate with referring partners to update, triage, and edit waitlist

Tracking hospitalization documentation from area hospitals
Minimum Required Skills & Qualifications:

Proficiency in Microsoft suite products including word, excel, one drive, power point, and outlook.

Excellent customer service and interpersonal skills

Candidates must be detail-oriented and highly organized

Self-directed and able to work with minimal supervision
