Primary Location: Natalie's Place
Natalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. Cardea Health’s transitional housing programs collaborate with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Natalie’s Place provides comprehensive supportive services, including onsite nursing and caregiving support, transportation to medical appointments, nutritious meals, housing navigation and placement assistance, and additional wraparound services designed to promote stability, recovery, and long-term housing success.
Job Description:
We are seeking professional, punctual, and responsible individuals to work in a driver as part of a multidisciplinary team at a medically supported shelter program serving people experiencing homelessness who are recovering from recent hospitalization, injury, or illness. Drivers are responsible for transporting new clients into the shelter setting at time of referral as well as bringing established clients to and from medical appointments and procedures. Drivers are expected to work closely with shelter nurses and housing staff facilitate quality and timely care of shelter residents. This position requires the ability and desire to work with a diverse group of individuals in a respectful and professional manner.
Job Duties:
- Communicate with clinical staff to schedule and safely transport clients, obeying all traffic laws, for medical appointments, program admissions and discharges, as directed by the clinical director
- Assist with the safe loading and seating of clients onto and off the agency vehicle, making certain that all residents are properly secured before drivin Assist with food transportation and deliveries to our various locations across the Bay Area as needed
- Conduct daily inspection and safety checks on the vehicles before the first use and following the final use of the day. Schedule regular maintenance as necessary
- Ensure that the vehicles are in proper operational order, registration and insurance information is present and up-to-date and promptly report any/all safety, mechanical or maintenance issues to supervisor
- Maintain communication with the program through use of a work cell phone or other means to always keep in contact on client location and status as necessary
- Maintain confidentiality of all services. Set and maintain appropriate boundaries with clients
- Pick up medications, document in medication log, and deliver to clinical staff as directed
- Always demonstrate a positive and professional image through dress and behavior. Display tack and friendliness when dealing with clients
- Participate in staff trainings and staff meetings
- Activate 911 in the case of medical or psychiatric emergency
- During time periods where there are no current client transportation needs, drivers are expected to assist clinical staff with daily non-medical tasks as requested
- Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.
