Job Description:
Charge Nurse (RN)
Department: Nursing
Reports To: Director of Nursing (DON) / Assistant Director of Nursing (ADON)
FLSA Status: Non-Exempt (Hourly)
Position Summary
The Charge Nurse is responsible for overseeing the daily nursing care provided to residents within the assigned unit while ensuring quality care is delivered in accordance with federal, state, and local regulations, facility policies, and professional nursing standards. The Charge Nurse provides leadership to Certified Nursing Assistants (CNAs), Certified Medication Technicians (CMTs), and other nursing staff while promoting a safe, compassionate, and resident-centered environment.
Essential Duties and Responsibilities
- Provide direct nursing care to residents according to physician orders and individualized care plans.
- Supervise and direct CNAs, CMTs, and other assigned staff during each shift.
- Perform comprehensive nursing assessments and monitor residents for changes in condition.
- Administer medications, treatments, and intravenous therapies as permitted by licensure.
- Accurately document nursing assessments, interventions, physician communications, and resident responses in the electronic medical record.
- Communicate changes in resident condition promptly to physicians, families, and interdisciplinary team members.
- Respond appropriately to resident emergencies and initiate emergency procedures when necessary.
- Complete admissions, transfers, discharges, and readmissions.
- Ensure physician orders are implemented accurately and timely.
- Review laboratory results, diagnostic reports, and follow-up care as ordered.
- Participate in development and revision of resident care plans.
- Monitor infection prevention and control practices throughout the unit.
- Ensure compliance with resident rights and maintain resident dignity, privacy, and confidentiality.
- Assist with wound care, catheter care, feeding tubes, and other skilled nursing procedures.
- Maintain narcotic counts and medication security according to facility policy.
- Complete shift reports and communicate resident status during shift change.
- Maintain adequate staffing assignments and assist with delegation of duties.
- Participate in quality assurance and performance improvement initiatives.
- Assist during state surveys and regulatory inspections.
- Ensure compliance with OSHA, CMS, HIPAA, and facility safety standards.
- Serve as a positive leader, mentor, and resource for nursing staff.
Qualifications
- Current, active Registered Nurse (RN) license in the State of Missouri.
- Current CPR/BLS certification.
- Long-term care or skilled nursing experience preferred.
- Strong leadership, communication, and organizational skills.
- Ability to prioritize multiple responsibilities in a fast-paced environment.
- Knowledge of state and federal long-term care regulations.
- Experience with electronic medical records preferred.
Physical Requirements
- Ability to stand and walk for extended periods.
- Frequently lift, transfer, push, or pull up to 50 pounds with assistance when necessary.
- Ability to bend, kneel, reach, and assist with resident mobility.
- Visual and hearing ability sufficient to safely perform nursing duties.
- Ability to respond quickly during emergencies.
