Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF)

$7,000 SIGN ON BONUS

Position Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a skilled nursing facility. The LPN works under the supervision of a Registered Nurse (RN) or physician to support resident health, administer treatments and medications, and ensure comfort and safety in a long-term care and rehabilitation setting.

Key Responsibilities

• Provide direct nursing care to residents according to care plans

• Administer medications and treatments as prescribed

• Monitor and document residents’ vital signs, conditions, and responses to treatment

• Report changes in resident condition to the RN, physician, or healthcare team

• Assist with admissions, discharges, and transfers

• Supervise and support Certified Nursing Assistants (CNAs)

• Maintain accurate and timely medical documentation in electronic medical records (EMR)

• Ensure compliance with infection control, safety, and regulatory guidelines

• Assist residents with activities of daily living as needed

• Communicate effectively with residents, families, physicians, and interdisciplinary staff

• Respond appropriately to emergencies and resident care needs

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of employment

• Graduate of an accredited practical nursing program

• Current CPR/BLS certification

• Previous experience in skilled nursing, long-term care, rehabilitation, or geriatrics preferred

• Strong communication and organizational skills

• Ability to work independently and collaboratively within a healthcare team

• Compassionate and resident-centered approach to care

Physical Requirements

• Ability to stand, walk, bend, lift, and assist residents throughout the shift

• Ability to lift up to 50 pounds with or without assistance

• Ability to respond quickly in emergency situations
