SNF CNA Job Description

Position Summary:
Provide compassionate, hands-on care to residents while promoting their comfort, dignity, safety, and independence. Assist nurses by observing and reporting changes in residents' conditions and documenting care provided.

Key Responsibilities:

• Assist residents with activities of daily living (ADLs), including:
  • Bathing
  • Dressing
  • Grooming
  • Toileting
  • Eating
• Transfer and reposition residents safely using proper lifting techniques and equipment.
• Take and record vital signs (temperature, pulse, respiration, blood pressure) as assigned.
• Answer call lights promptly and assist with resident needs.
• Help residents with mobility, walking, and range-of-motion exercises.
• Make beds and maintain clean, organized resident rooms.
• Observe and report changes in residents' physical, mental, or emotional condition to the nurse.
• Document care provided according to facility policies.
• Assist with meal setup, feeding, and monitoring food and fluid intake.
• Maintain infection control and safety procedures.
• Provide emotional support while treating residents with dignity and respect.

Qualifications:

• Current state CNA certification.
• CPR certification (if required by the facility).
• Ability to lift, transfer, and assist residents safely.
• Strong communication and teamwork skills.
• Compassionate, dependable, and patient-centered approach.

Physical Requirements:

• Frequent standing, walking, bending, and lifting (often up to 50 pounds with assistance).
• Ability to push wheelchairs and assist with transfers.
• Ability to respond quickly to emergencies.

Work Environment:

• Skilled nursing facility or rehabilitation center.
• Day, evening, night, weekend, and holiday shifts may be required.
• Regular interaction with elderly residents, rehabilitation patients, families, nurses, therapists, and physicians.

SNF CNAs often care for residents recovering from surgery, illness, injury, or chronic medical conditions, so the role typically involves more complex care needs than in many assisted living settings.
