- Responds to referrals and identifies high-need populations such as those with new diagnoses, terminal illnesses, homelessness, or complex social issues.
- Conducts psychosocial assessments, develops and documents care plans, and ensures timely record-keeping compliant with regulations.
- Assists patients and families with coping strategies, social support, and post-discharge planning, including resource referrals and community connections.
- Provides social medicine consultations for cases involving abuse, neglect, advance care planning, and legal decisions.
- Contributes to discharge planning, ensuring appropriate community placement and resource utilization.
- Engages in performance improvement, departmental activities, staff education, and quality assurance efforts.
- Minimum one year of healthcare experience.
- Master’s degree in Social Work and valid licensure as a Licensed or Clinical Social Worker.
- Basic Life Support certification within six months of hire.
- Experience working with interdisciplinary teams and understanding medical terminology.
- Ability to collaborate with healthcare providers, patients, and support systems effectively.
- Knowledge of care processes, community resources, family dynamics, and end-of-life issues.
- Strong customer service, organizational, and documentation skills.
