Job Summary:
Provides psychosocial assessments and social medicine services
as part of the emergency department/CDU care team to address patients' social, emotional, and support needs.

Key responsibilities include:
- Responding to referrals and case finding for high-need populations such as those with new diagnoses, terminal illnesses, homelessness, or complex social issues.
- Conducting psychosocial assessments, developing and implementing care plans, and documenting interactions according to standards and regulations.
- Assisting patients and support systems with coping strategies, discharge planning, and connecting them to community resources.
- Providing consultations on abuse, neglect, advance care planning, and legal decision-making.
- Participating in performance improvement initiatives, departmental activities, and staff development.
- Masters degree in Social Work and at least one year of healthcare experience.
- Licensed Social Worker or Licensed Clinical Social Worker.
- Basic Life Support certification.
- Skills in interdisciplinary collaboration, medical terminology, patient education, and community resource knowledge.
