Case Manager (Mental Health / Medicaid Services)

Employment Type: Full-Time

About the Role

Are you passionate about connecting people with the resources and support they need to thrive? We're looking for a dedicated Case Manager to provide comprehensive support to Medicaid clients — conducting assessments, building individualized treatment plans, coordinating care, and helping clients access the community resources that make a real difference in their lives.

In this role, you'll be a key point of connection between clients, families, and service providers, ensuring continuity of care every step of the way, all while upholding high ethical and professional standards.

What You'll Do

Assessment & Treatment Planning

  • Complete CANS assessments for Medicaid clients under 18 every 90 days
  • Develop individualized treatment plans aligned with therapeutic goals every 90 days
  • Regularly review and update plans based on client progress and changing needs
  • Monitor adherence to treatment recommendations and follow up on missed appointments

Care Coordination & Collaboration

  • Coordinate services with therapists, psychiatrists, medical providers, schools, and community agencies as clients need
  • Facilitate communication between clients, families, and providers to ensure continuity of care
  • Advocate for client rights and help clients access healthcare, educational, housing, and legal resources
  • Help clients navigate Medicaid/Medicare, disability benefits, and community programs
  • Participate in interdisciplinary team meetings, case consultations, required supervision, and weekly staff/staffing meetings

Client Support & Skill Development

  • Teach and reinforce daily living skills based on clinical needs
  • Support clients during crisis situations with de-escalation assistance
  • Coordinate emergency services, crisis lines, and safety planning when needed
  • Provide stabilization support and ensure smooth transitions to higher levels of care when appropriate

Documentation & Compliance

  • Track client progress and document all contacts, interventions, and updates in Simple Practice (EHR)
  • Complete required reports, billing documentation, and case notes accurately and on time
  • Maintain client confidentiality and adhere to HIPAA, ethical guidelines, and legal standards

Community Partnership & Outreach

  • Build and maintain relationships with community partners, referral sources, and support programs
  • Educate families and community agencies about mental health resources and case management services
  • Engage in ongoing professional development to stay current with best practices

What You'll Bring

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
  • Experience working with children, families, or mental health services highly beneficial
  • Strong understanding of community resources and service systems (healthcare, education, housing, etc.)
  • Skilled in crisis intervention, de-escalation, and supportive communication
  • Strong organizational skills and ability to manage a caseload effectively
  • A client-centered approach and strong advocacy skills

Pay: $18.00 - $25.00 per hour
