Care Coordinator II – Substance Use Disorder Treatment

We are seeking a Care Coordinator II to play a critical role in ensuring seamless, compliant, and patient-centered care from admission through discharge.
Position Summary
The Care Coordinator II serves as the central hub of communication and coordination for each client’s treatment journey—from intake through discharge planning. This role ensures continuity of care, accurate documentation, and alignment with individualized treatment goals while collaborating across clinical, medical, and operational teams.
This position is essential to maintaining compliance, supporting patient engagement, and driving strong clinical and operational outcomes.
Essential Duties and Responsibilities
Care Coordination (Core Function)
- Serve as the primary liaison between clients, interdisciplinary team members, referral sources, and community providers
- Maintain timely, accurate documentation of care coordination activities, treatment plan updates, and payer requirements
- Support development of individualized treatment goals and connection to internal and external recovery resources
- Track payer authorizations, continued stay criteria, and utilization review requirements
- Proactively communicate client needs, risks, and updates to clinical, medical, and leadership teams
- Participate in clinical on-call rotation
Intake & Admission
- Conduct screenings and support biopsychosocial assessments within scope
- Facilitate new client orientation, including program expectations, schedules, and patient rights
- Ensure completion of all intake documentation, consents, and EMR requirements
- Coordinate with medical team for physician orders, labs, and initial evaluations
- Support a trauma-informed, structured admission experience
Discharge Planning
- Initiate discharge planning at admission and integrate into the treatment plan
- Coordinate aftercare referrals (IOP, MAT providers, sober living, housing, peer supports)
- Review discharge instructions, medications, and follow-up appointments with clients
- Complete discharge documentation in compliance with regulatory timelines
- Facilitate warm handoffs to community providers
Crisis & Behavioral Support
- Recognize and respond to crisis situations using de-escalation and safety protocols
- Support development of individualized behavior plans when needed
- Document all crisis interventions and coordination efforts
Compliance & Quality
- Ensure all processes align with Joint Commission, state licensure, Medicaid, and 42 CFR Part 2 standards
- Maintain strict confidentiality and professional boundaries
- Participate in audits, quality improvement initiatives, and corrective actions
- All of duties as assigned.
Qualifications
Required
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field
OR minimum of two (2) years of experience in SUD/behavioral health in lieu of degree - Minimum of two (2) years of experience in a behavioral health or substance use treatment setting
- Strong understanding of care coordination, documentation, and interdisciplinary collaboration
- Ability to manage multiple priorities in a fast-paced environment
Preferred
- Experience with intake, discharge planning, or utilization review
- Familiarity with ASAM criteria and stages of change
- Experience with EHR systems (e.g., Lightning Step)

Schedule
- Full-time position; ability to take on call; schedule may include evenings, weekends, or rotating coverage based on facility needs
