General Description
The Case Manager I provides comprehensive case management services for individuals with behavioral health and substance use needs. This position coordinates care across clinical, medical, and community systems to ensure each client’s treatment goals and support needs are met. The Case Manager I works collaboratively within a multidisciplinary team to support engagement, stability, and recovery.
Essential Duties & Responsibilities
- Provide case management for clients with moderate to high needs, including crisis coordination and discharge planning.
- Facilitate interdisciplinary case reviews and communicate progress, barriers, and next steps.
- Maintain collaborative relationships with community agencies, courts, and referral sources.
- Assist clients in accessing housing, employment, benefits, and recovery support resources.
- Maintain timely, accurate documentation in the EHR.
- Participate in performance improvement and compliance initiatives.
- Perform other duties as assigned.
Qualifications
Required:
- Three (3) years of behavioral health experience with demonstrated case management competencies.
- Working knowledge of ASAM Criteria and Stages of Change.
- Proficiency in electronic health record documentation.
- Current CPR/First Aid certification or ability to obtain within 30 days.
Preferred:
- Bachelor’s degree in social work, psychology, counseling, or related field OR three (3) years of behavioral health experience with case management competencies.
- Two (2) years in residential or outpatient SUD/co-occurring programs.
- Training in motivational interviewing or trauma-informed care.
- CADAC or related credential.

Work Conditions
- Work performed in a behavioral health or residential treatment environment.

- Schedule may include evenings, weekends, or holidays as needed.

- Ability to sit, stand, and walk for extended periods; lift up to 25 lbs.
- Adequate vision and hearing to respond to client needs.
- Ability to concentrate, problem-solve, and manage multiple priorities.
- Ability to maintain professionalism and composure during crises.
