The Alumni Coordinator is responsible for developing, implementing, and sustaining a
comprehensive alumni program that supports patients transitioning into long-term recovery.
This role plays a critical part in improving patient engagement, strengthening continuity of care,
reducing readmissions, and supporting long-term recovery outcomes. The Alumni Coordinator
serves as a bridge between discharged patients and ongoing recovery support services, ensuring
a consistent, patient-centered approach aligned with clinical best practices and regulatory
expectations.
This position partners closely with Clinical, Operations, and Business Development teams to
build a strong recovery community, reinforce aftercare planning, and support value-based care
initiatives.

🗝️essential Duties & Responsibilities
Alumni Program Development & Management
- Design, implement, and maintain a structured alumni program for discharged patients
- Develop and manage an alumni engagement calendar, including groups, events, and outreach campaigns
- Maintain and track alumni participation through CRM and/or EMR systems
- Establish consistent outreach cadence (e.g., 30, 60, 90-day and ongoing follow-up)
- Build scalable processes for alumni engagement across the organization Patient Engagement & Retention
- Conduct post-discharge follow-up calls and outreach to support ongoing recovery
- Facilitate alumni support groups (in-person and virtual)
- Coordinate peer mentorship opportunities between alumni and current patients
- Support relapse prevention efforts through consistent engagement and connection
- Promote continued participation in recovery-focused programming
- Lead groups in the facility on alumni involvement, service roles and the importance of
- Partner with clinical team to support discharge planning and aftercare coordination
- Ensure alumni are connected to appropriate recovery resources, including:
- Outpatient services (IOP/OP)
- Recovery support groups (e.g., 12-step, SMART Recovery)
- Medication-Assisted Treatment (MAT) providers
- Reinforce individualized continuing care plans post-discharge
- Support warm handoffs and transitions across levels of care
- Track and monitor alumni engagement and recovery metrics, including:
- Serve as the primary admin for the alumni engagement app; manage access, notifications, rewards, virtual meetings
- Monitor engagement analytics and pull monthly reports covering attendance, check-in rates, referrals and KPIs
- Review reports with leadership and adjust outreach strategy accordingly
- Post-discharge engagement rates
- Readmission trends (30/60/90-day)
- Support data collection aligned with value-based care initiatives and quality measures
- Prepare and provide regular reports to leadership on program performance
- Plan and execute alumni programming, including:
- Monthly alumni meetings
- Sober social events
- Annual Alumni events or reunions
- Coordinate monthly graduation/speaker meetings
- Coordinate family engagement opportunities when appropriate
- Foster a strong, inclusive recovery community culture
- Partner with community organization for sober events
- Establish and maintain cultural touchstones: milestone recognition walls, name plaques, brick pathways, or other lasting commemorations
- Establish Alumni Committee
- Develop the Committee into a self-sustaining leadership body that reduces coordinator dependency
- Collaborate with Business Development to strengthen community relationships
- Support referral pipeline through alumni engagement and success stories (as appropriate and compliant)
- Assist in building partnerships with community recovery resource
- Ensure all outreach and engagement activities comply with:
- HIPAA
- 42 CFR Part 2
- Document alumni interactions in accordance with organizational policies
âś…Qualifications & Knowledge
- Personal Experience with recovery – ideally a graduate of program or similar program
- Strong understanding of recovery-oriented systems of care
- Deep, authentic buy-in to the mission
- Knowledge of patient engagement strategies across stages of change
- Ability to build relationships and engage diverse patient populations
- Strong organizational, communication, and facilitation skills
- Proficiency in Microsoft Office and data tracking systems
- Valid driver’s license with acceptable driving record
- Knowledge of ASAM Criteria
- Familiarity with 42 CFR Part 2 requirements
- Experience with CRM or EMR systems for tracking engagement
- Understanding of value-based care and patient outcome metrics
Required
- High School Diploma or equivalent; 2 years sustained recovery
- Bachelor's degree in behavioral health, Social Work, Psychology, Marketing, or related field
- 2–4 years of experience in:
- Substance Use Disorder treatment
- Behavioral health or community outreach
- Alumni relations, case management, or recovery support services
- Experience working within ASAM Levels 3.5 and/or 3.7 settings
Environment:
- Indoor residential treatment setting with moderate activity level.
- Frequent standing, walking, bending, and lifting up to 50 lbs. with assistance.
- Ability to assist clients in physical tasks and respond to emergencies.
- Ability to prioritize multiple tasks in fast-paced settings.
- Maintain professionalism with clients experiencing withdrawal, emotional distress, or crisis.
- Sustain focus for documentation and monitoring responsibilities.
